Manifest Instructions

The Uniform Hazardous Waste Manifest form allows entry of information to create official manifests for hazardous waste generators, transporters and disposal facilities. A manifest can only be printed when it is initially created.

You must have either the "Generator" role or the "Transporter" or the "TSDF" role assigned to your user profile to create an on-line manifest. A transporter, generator, or TSDF may contact DTSC via email at hwtsadmn@dtsc.ca.gov if they experience problems.

Generators (Top area)

The top section of the generator (blue) portion of this form contains several fields. See the manifest instructions for specific assistance.

Please note: All fields marked with a red asterisk (*) are required fields.

Generators (section 2)

Waste stream information.

To select either a RCRA or a Non-RCRA code:

1. Click the letter ("a", "b", "c", or "d") for the corresponding location you want to place the waste item.
2. This will move you to the Resource Conservation and Recovery Act (RCRA)/non-RCRA area of the form.

3. Here you should select either a RCRA code From the drop down menu, or click the "Non-RCRA Solid" or "Non-RCRA Liquid."
4. Clicking on Non-RCRA Solid/Liquid will enter the statement in the next text box. If you need to, modify the text.
5. If you check N.O.S. or RQ, you will have to enter the N.O.S. or the RQ text.
6. When you are ready, verify the "Add to row" is correct and click "Add."

To Clear a RCRA or a Non-RCRA code.

1. Be sure the "Add to row" shows the row letter you want to clear.
2. Click "Clear."

Please note: Generators do not fill out the "K" Section of the blue generators section. See graphic:

Transporters: Fill in the Green portion.

TSDF (Treatment, Storage, and Disposal Facility): Fill in Yellow and Section K from Blue.

When you submit your Manifest:

When you are ready to complete and submit your manifest you have two choices, "Submit and Close" and "Submit and Create Continuation."

When you submit and close the following occurs:
• Pre-processing checks to verify that all required fields have been entered.
• Any errors are presented in a pop-up window.


• Click "Close" to return to manifest and correct your errors.

If a successful submission occurs:
• A new screen appears with your twelve-character "Manifest Number."
• The official manifest is launched in Adobe Acrobat Reader for you to print.
• If you selected "Submit and Create Continuation," you will be presented with a continuation.
